6. The Trust’s Patient Safety Incident Response Plan: locally pre-defined PSII (local priorities based on the risk profile)

6.1 A review of data and soft intelligence from the sources outlined in point 3 above, & the resulting risk profile has highlighted the following local priorities for PSII.

6.2 The above priorities are predefined & are subject to routine review to account for emerging risks and the strength of the relevant & ongoing improvement work. A formal review will be undertaken in June 2025.  It is important to note that not all incidents listed above as local priorities will automatically be investigated as PSII.  Whether an incident is investigated as a PSII or not will depend on the circumstances surrounding the individual incident and whether or not there is new opportunities for learning. The categories above are intentionally broad, to enable decisions to be made on a case by case basis, there by fulfilling the proportionate response guidance.
