9. Roles & responsibilities: Governance & Oversight
9.1 Daily incident reviews are undertaken at unit level & by the Patient Safety Team & immediate actions are taken.
9.2 PSIIs decisions will be made by the Executive Team & all other learning response decisions will be made at directorate level by the Directorate Leadership team.
9.3 A weekly summary of Directorate level decisions on learning will be pulled together by the Patient safety Team and shared on a weekly basis with the executive team.
9.4 A monthly report on learning & improvement themes will be presented to the Clinical Safety & IQAG as part of the quality performance report.
9.5 A quarterly report will be presented to the Quality Committee & ICB.
