Croydon Stars is an annual recognition awards programme organised by Croydon Health Services NHS Trust, showcasing the great work of it's staff, volunteers and those it works in partnership with across the borough.
This year, in our aim to acknowledge more of our Croydon place colleagues, we are introducing the brand new Excellence in Primary Care award.
Category criteria
Nominees (a team or individual) will have demonstrated exceptional commitment to providing comprehensive care to their community and excelled in working collaboratively and innovatively with other healthcare professionals to provide integrated and holistic care to their patients. They will have evidenced extraordinary compassion and empathy in their interactions with patients and improved patient care and outcomes within their community.
Who can nominate?
We welcome nominations from colleagues working in primary care field, along with those working for other professional partners across the borough.
How to nominate
Complete the form below by the deadline date. Each nomination will require at least one written example showcasing the category criteria (see above) to increase it's chances of being shortlisted by the judges. What happens when the nomination period ends?
Once all nominations are in, they will be reviewed by a judging panel made up of colleagues from the GP Collaborative and a range of professionals from Croydon Health Services NHS Trust. The shortlist will then be released in early autumn, and the winner revealed at a special awards ceremony later in the year, which will reveal this category winner alongside awards recognising leadership, teamwork, compassionate care and those working behind the scenes to name just a few as well as the publicly voted Star of the Year award.
