The NHS is working to connect your care across South West London.

Soon healthcare professionals will be able to access your records from other health and care services to help them make better decisions about your care.

This will make it quicker and easier whenever your visit your GP or local hospital.

It means that people such as your GP, hospital doctors and nurses will be able to see information about you through a secure system – which can help speed up your treatment and can even save lives.
