Adult Domiciliary Physiotherapy Service provides physiotherapy service for patients unable to be seen as an outpatient  

Service overview

The Domiciliary Physiotherapy Service sees patients who are housebound or who are best seen at home because of a need to assess function in own environment.

Patients may be referred following an acute episode (e.g. fracture, illness or surgery) or because they are managing long term conditions, or are elderly frail and experiencing mobility difficulties

Interventions vary from a single visit to provide equipment or advice, to a defined episode of care (e.g. following surgery), or to several episodes of care over a longer period (e.g. for a patient managing a deteriorating condition such as Multiple sclerosis)

Management may include:

• Exercise programme
• Mobility advice
• Provision of equipment
• Advice/guidance

We treat

Conditions may be orthopaedic, neurological, or medical. Patients often have multiple pathologies.

Referral criteria

• Client has a Croydon G.P.
• Aged 16 or over.
• Housebound (i.e. unable to leave home)  or better seen at home because of a need to assess function in own environment
• Client does not require a specialist service which is outside the remit/skill set of the team (e.g. Lymphoedema management, Women’s Health)

LIFE (Living Independently for Everyone) Tel: 020 8274 6974

LIFE provide intensive, often daily, multi-disciplinary (OT and Physiotherapy) rehabilitation input over a time limited period not exceeding 6 weeks.

CNRT (Community Neuro Rehab Team) Tel: 020 8274 6880

CNRT provide intensive multi-disciplinary rehabilitation input over a time limited period not exceeding 13 weeks for patients with neurological conditions.
