Children's Hospital at Home (CHAH)

The Children’s Hospital at Home Team is an integrated team of community children’s nurses, dramatherapists, nursery nurses and administrative support that provides a service for children at home with additional health needs.

The service provides support across Croydon for babies, children and young people either at home, in school or clinic setting, including:

  • Acute nursing care to reduce the need for hospital admissions and supprt early dischrge when they do have to go to hospital
  • Ongoing case management and care
  • Co-ordination of care for children with long term and complex conditions
  • Teaching and education packages to children and families so that they can independently manage their health needs
  • Palliative care for children with life threatening and life limiting conditions
  • Providing choice of place of care for those who are at the end of life
  • Play therapy
  • Support for children, young people and siblings through ‘Willow’ children’s bereavement service
  • Nurse Specialist service for children with Diabetes  

The specials school service provides direct nursing care to children aged 4 – 19 years who have additional medical needs and attend special schools within Croydon. The main site is St. Giles and Redgates school in South Croydon.

As our nurses do not carry supplies, if you require nursing
care such as wound dressing, you will need to obtain any supplies required to
give your care from your GP prior to the visit.
