Referring your patients to Croydon Health Services
22 February 2017

We want to make it much easier for you to book appointments at the Trust and access information about your patients, so have grouped many of our specialist services together in PODs (or Points of Delivery teams).

When referring your patients to any of the specialist services listed below, please call or email the PODs directly where a dedicated team will be available to help you.

All PODs will be open from 8.30 to 17.30, Monday to Fridays.

To make these improvements, many of our telephone numbers and email addresses will change.  We will do this gradually over three weeks.

From 27 February, to refer your patients to any of the following specialties please use the new POD contact details below.

Please print the following update and share this with your practice teams.

From Monday 27 February.

Croydon Health Services NHS Trust provides integrated hospital and community services throughout the borough to help prevent ill health and provide expert medical care when needed.

The Trust runs two hospitals and our experienced district nursing teams and community matrons cover every corner of the borough to care for adults and young people.  Our Children’s Hospital at Home which provides care for children with long-term conditions at home – without having to travel to hospital.

Croydon University Hospital (CUH) performs 26,000 surgical operations every year, and provides more than 100 specialist services, including conditions affecting the heart, cancer care and treatment for musculoskeletal disorders.  CUH also offers 24/7 maternity services, including labour ward, midwifery-led birth centre and the Crocus home birthing team.

Croydon’s one Emergency Department is based at Croydon University Hospital.  We see more than 400 people through our Emergency Department and urgent care services.  From April 2017, Croydon residents will have access to a seamless network of urgent care services, run in partnership with the Croydon GP Collaborative and AT Medics, including booked urgent appointments with a GP, available from 8am to 8pm, seven days a week by calling NHS 111.

Purley War Memorial Hospital offers easy-access to outpatient care, including diagnostic services such as blood tests and x-rays, physiotherapy and ophthalmology services run by Moorfields Eye Hospital.

The Trust also provides a 24/7 Rapid Response Service which means adult patients can be assessed in their own homes by our specialist teams within two hours of being referred by their GP.
