Service overview

14 bedded Acute Care of the Elderly (ACE) Short Stay Unit, providing comprehensive geriatric assessment by a multidisciplinary team.

Rapid access Acute Care of the Elderly (ACE) Clinic (CUH Monday to Friday)


We treat

The ACE Clinic and Hotline/Advice service aims to support and care for elderly patients from the emergency department or from primary care. It is a consultant-led service that embraces a multidisciplinary approach to improve quality of care and patient experience.

The ACE service offers GPs guidance on the management of elderly patients via a GP hotline and rapid access assessments for appropriate referrals.


All referrals should be discussed with the ACE Consultant in the first instance via the GP hotline.


Patients accepted for review will be seen by a Consultant Geriatrician or Associate Specialist as well as a specialist nurse, therapist, care manager and voluntary sector input as appropriate.


To ensure smoother patient care GPs should provide the following details:


Patient's or next of kin telephone number

Background information

Whether the patient is arranging transport

(Transport can be arranged by Croydon Health Services if required)


Referral criteria

GPs may discuss patient care and potential referral for patients meeting the following criteria:

  • Aged 65 years and over
  • Patients with acute/sub-acute medical problems who may not need 24 hour medical and nursing in-patient care
  • Complex, frail, and high risk of approaching crisis/requiring admission
  • Abnormal blood test


Less appropriate for referral:

  • Suspected fracture
  • Haemodynamically unstable needing to be blue-lighted to the emergency department 
  • Acutely unwell and sick clearly needing 24 hour medical and nursing in-patient care
  • Any acute condition that needs urgent triaging and cardiac monitoring such as suspected ACS
