What do I do if I want to change or cancel an appointment?

Telephone the service.

What should I bring to my appointment?

Glasses and hearing aids if required.

Patients are also welcome to bring a family member, friend
or carer to accompany them to the appointment.

You will be asked to complete some questionnaires giving
important information about your condition. They will accompany your appointment letter. Please bring them along with you to
your appointment.

What time should I arrive for my appointment?

Please arrive on time for the appointment as
stated in the appointment letter.
