What do I do if I haven’t received my appointment yet?

Check where and when your dentist referred you then contact the department to check that the referral has been received and accepted.

What should I bring to my appointment?

• Your completed medical history and registration forms.
• A list with the names of all the medicines you take
• Any dental appliances or radiographs you have that are relevant to your problem

Please arrive on time and allow sufficient time to register at the desk.
