The heart failure nursing service delivers individualised care to people in Croydon with chronic heart failure. It provides case management for the most complex patients and co-ordinates end of life care when required. Clinics are held at Sanderstead Clinic and Norbury Health Centre, and we see patients at home.

What should I bring to my appointment?

We will need to know about illnesses you have had in the past as well a list of all the medicines you take including any that are not prescribed by your GP.

What time should I arrive for my appointment?

We endeavour to see all patients as near to their appointment time as possible. If circumstances lead to a 30 minute delay, we will try to contact you to let you know.
