The HVOP Team is based at Shirley
Clinic and Norbury Health Centre

The HVOP Team assesses need, identifies services required and develops and implements a plan, to maximize optimum health and independence. 

Services offered:

  • Identified needs – isolated, frail, vulnerable older people, who are deemed not coping 
  • Falls and accident prevention – prevention of hospital admission 
  • Recent bereavement and/or carer stress 
  • Assessment for equipment, supporting independence 
  • Health promotion and education 


The HVOP Team provide comprehensive and detailed support on self care: including falls prevention, mobility, benefits advice, home safety, nutrition, skin care,  social networks and support (e.g. Lunch Clubs and Day Centres), mobility and equipment aids, medication, exercise, smoking cessation, pendant alarms, housing options, travel services (e.g. Blue Badge and Taxi Card) hearing and sensory impairment resources, befriending, handyman service, continence assessments, care agencies, balance specialist, medical loans and surgical appliances, community dentist, foot care and

podiatry, home library and hairdressing services, home energy – heating and
efficiency, community opticians and private home meals delivery services.
The team also provides support and information to carers including links to Carers
Network.
