Diabetes - Community
The Croydon Community Integrated Diabetes Service (CCIDS) is a new intermediate diabetes service for people living in the borough of Croydon. While most routine diabetes care is provided by your local GP practice, CCIDS provides a range of additional services. These include:
- a central point of contact for information about all aspects of diabetes care
- a comprehensive programme of education for people with Type 1 and Type 2 diabetes, their families and carers.
- supporting people with diabetes who are experiencing difficulties with their blood glucose control
- local specialist dietetic clinics
- local diabetes specialist nurse-led clinics including a weekly urgent assessment.
Anyone can contact CCIDS for advice or information. If you would like to be seen in one of the specialist clinics, or to attend an education programme, you will need to seek a referral from your GP. All Croydon GP practices should be familiar with the referral process.
CCIDS is also dedicated to supporting and providing education and mentorship to fellow health care professionals who would like to improve the standard of diabetes care that they provide. We aim to provide regular courses, study days and updates for GPs, practice nurses, community nurses and staff working in nursing and residential homes.

Croydon Diabetes Network
CCIDS is working closely with Croydon Diabetes Network, which connects everybody involved with diabetes services in the borough. This includes healthcare professionals, people working in related fields such as older people’s services and people with diabetes, their families and carers.
The network publishes a newsletter and maintains a website (www.croydondiabetes.org.uk), runs events to increase awareness around diabetes issues and works with other organisations involved in diabetes care.

Validated patient education programmes:
- Type 1 Diabetes Education: Dose Adjustment for Normal Eating (DAFNE)
- Type 2 Diabetes Education: X-PERT programme
- X-PERT insulin programme (new service)
- Byetta initiation / weight management groups (new service)
Accredited education programmes for health care professionals:
- Warwick University Certificate in Diabetes Care
- Warwick University Intensive Management of Type 2 diabetes
The services also runs a number of non-accredited education programmes for health care professionals, including the health care assistant programme.
Clinics
- Diabetes specialist nurse-led clinics
- Diabetes specialist dietitian-led clinics
- Consultant diabetologist outreach clinics
Appointments
Following referral to CCIDS you will receive a letter inviting you to contact CCIDS to arrange an appointment at your convenience. You will then receive written confirmation of your appointment, together with a map which will give you directions on how to find us.
You will also be advised about what to bring with you.
If you are attending a diabetes specialist nurse clinic you will be requested to bring your diabetes hand held record if you have one, or your blood glucose monitoring diary.
If you are attending a dietetic clinic you may be requested to keep a food diary for a few days prior to your appointment.
