Cardiac Rehabilitation - Community

The Community Cardiac Nurse Specialist team provides care to help patients recover following an acute cardiac event or intervention, such as heart attack, angioplasty, stent or cardiac surgery.

The aim of the service is to support patients to regain their quality of life and return to as normal life as possible, through appropriate access to health care and education about self-care for their condition. Where appropriate, patients are cared for using a case management approach in supporting them to live with and manage long term cardiac illness. We offer lifestyle information and intervention to improve health and wellbeing, as well as the opportunity to attend cardiac rehabilitation exercise and education classes at Croydon University Hospital if appropriate.

The service also assists general practice and community health care professionals in the general care of cardiac patients through the provision of guidelines, education and professional advice. We contribute to the strategic planning of service development for cardiac patients and act as an expert resource for colleagues in primary care.

What should I bring to my appointment?

You will have been sent a health questionnaire so please could you bring this completed, along with your current medication list and any letters or information booklets you were given when you left the hospital. If you’re diabetic you will also need your glucose monitoring book 

What time should I arrive for my appointment?

The team are usually very prompt with appointment times.
