Cardiac Rehabilitation
All patents admitted to Croydon University Hospital following a heart attack or who have required angioplasty and stenting for heart disease will be seen by a Cardiac Rehabilitation Nurse prior to discharge.
They will be assessed and offered individualised information on the nature of the disease, returning to normal activities, diet, risk factors, medication and long term management of their condition.
Patients are then referred to our community colleagues for support in the early discharge period before being referred for an assessment with one of our exercise professionals. We currently run four separate classes a week of different intensities where patients will be taught how to exercise safely and are given an individual exercise prescription.
All programmes highlight the importance of behaviour change during the education component and include a relaxation element. Following completion of the programme patients are encouraged to continue exercising in the Croydon area at venues staffed by specifically trained exercise professionals.

A service where patients who have been admitted to the Peter Stubbs Cardiac Zone following a recent heart attack or following angioplasty/stenting are offered information and support prior to discharge
Cardiac Rehabilitation Programme
A programme of exercise, education, and relaxation held in Shirley House, Croydon University Hospital for the patients set out above that have been discharged from hospital as well as any Croydon resident who have had treatment, including Coronary Artery Bypass Grafting, or other heart surgery at a hospital outside the Croydon area.
PCI Clinics
A Nurse Led Discharge Clinic which runs alongside the Cardiology Clinic where patients who have successfully exercised in the Cardiac Rehabilitation Programme are discharged from Cardiology back to the care of their GP

The Croydon Cardiac Support Group is for patients and carers. It is run by ex-heart patients who know just how you may be feeling at this time. They visit the Peter Stubbs Cardiac Zone once a week and hold meetings in the Post Graduate Medical Centre in Croydon University Hospital every second Wednesday of the month from 7.30-9.oopm. Information after discharge
If you are discharged from Croydon University Hospital you should find information about the type of Outpatient Appointment you will receive on the bottom of your discharge letter.
If your discharge letter states Cardiology OPA then you will receive a letter in the post for a Cardiology Outpatient Appointment.
While you are attending The Cardiac Rehabilitation Programme we will confirm that you have received this letter and make appropriate enquires if you have not.
If you are discharged from Croydon University Hospital and your discharge letter states PCI Clinic Appointment then you will be discharged by the Cardiac Rehabilitation Nurse who will monitor your progress while you are exercising in the Cardiac Rehabilitation Programme before giving you this appointment

You will probably require an ECG prior to your appointment so please allow time for this.
