Community Intermediate Care Services
The service aims to provide person-centred care by working in partnership with the patient and carer to enable the patient to regain and maximise their full potential, as their condition allows, with the aim of discharging them from the service within a 6 week period.
- Provide access to multidisciplinary assessments appropriate to the patient’s needs for all patients referred and admitted to the service
- Effectively manage admissions to and discharges from the CICS caseload, including the intermediate care beds, so that capacity within the service is maximised
- Work in partnership with patients and their carers to negotiate and agree nursing and therapy care plans and rehabilitation goals
- Work in collaboration with other agencies to meet patient needs effectively
- Liaise with colleagues in primary, secondary and tertiary care
- As part of the Community Stroke team to provide rehabilitation for patients with social care needs who have potential to improve
- As part of the Integrated Falls Service to provide rapid response physiotherapy assessment for patients who have fallen or are at risk of falling
- Provide intervention that is evidence based and appropriate to the patient’s needs
- Identify patients that may require specialist referrals and assessment and action these
Criteria
Exclude:
- People under the age of 18 years
- People whose needs would be more appropriately met by other community services and/or Social Services/voluntary services
Referral Process
As part of the reorganisation of Adult Community Nursing Services, CICS will be moving at the beginning of September from its current base at Lennard Road to:
- Waddon Clinic (South CICS Team)
- Woodside Health Centre (North CICS Team)

As of 22/8/11, all referrals for CICS will be managed via the Single Point of Referral (SPOR) for District Nursing.
Please note the following:
Croydon University Hospital Ward Referrals to CICS
- Please continue to complete these electronically via the Traffic Lights System.
- Please fax OT, physio and any other relevant reports to SPOR.
- SPOR will inform referrers which CICS Team will follow up the referral (North or South) and will provide contact details for the relevant team.
- North or South CICS Team will then contact the referrer to discuss the referral, arrange CICS assessment on the ward if required.
All Other Referrals to CICS
Monday – Sunday (incl BH) 08.30 – 16.30:
- Please fax your referral, therapy reports and any other relevant information to SPOR.
- SPOR will inform referrers which CICS Team will follow up the referral (North or South) and will provide contact details for the relevant team.
- North or South CICS Team will then contact the referrer to discuss the referral, arrange CICS assessment either on the ward or at home.
Monday – Sunday (incl BH) 16.30 – 21.00
- For urgent referrals requiring an immediate response, please page CICS on 07659 145123, leaving a contact number before being faxing the referral to SPOR.
- For non-urgent referrals, requiring a response within 24 hours, please fax the referral to SPOR.

Waiting Times
- Referrals will be screened and prioritised on the day they are received
- Referrals for CICS intervention to support a hospital discharge are responded to in 24 hours of referral
- Referrals for hospital avoidance are responded to within 4 hours of referral
