Community Heart Failure nurse specialist service
The British Heart Foundation Community Heart Failure Nurse Specialist Service provides supportive care for patients with a diagnosis of heart failure in Croydon.
Appropriate patients are cared for using a case management approach in supporting them to live with and manage long term cardiac illness. Provide information to people living with heart failure to enable self care.
- Agree management and/or care plans with patients, their carers and others involved in their care
- Support of carers to people with heart failure
- Improvement of other health and social care professionals’ understanding and confidence in managing the signs and symptoms of heart failure
- Support patients with complex needs through case management
- Ensuring that appropriate patients with heart failure receive optimum tolerated levels of pharmacological therapy
Criteria
Exclude:
- People under the age of 18 years
- Patient lives out of area or has an out of area GP
- Patient does not have objective evidence of cardiac dysfunction as a cause of heart failure.
- Patient refuses service
Referral Process
- Hospital discharge – referral through HHFNS
- Discharge from secondary care clinic
- GP referral & Community healthcare professional referral - see form on the right of this page
Essential information to be included on all referrals:
- Demographics
- Current medications
- Medical history including echocardiography information
- Latest blood test results
- Reason for referral

Waiting Times
Telephone assessment within 48 hours of referral being received:
- Hospital discharge newly diagnosed patient with heart failure
- Known heart failure referred because of clinical deterioration
Telephone assessment within 2 weeks of receiving referral unless identified as being unstable:
- Outpatient newly diagnosed patient with heart failure
- Known heart failure referred for additional support or information to support self care
