Cardiac Rehabilitation and Secondary Prevention
This service is co-provided with inpatient cardiac rehabilitation colleagues at Croydon University Hospital for patients recovering from an acute cardiac event or intervention.
Where appropriate patients are cared for using a case management approach in supporting them to live with and manage long term cardiac illness.
Case Management
Patients who require further assistance with health due to more complex needs and co-morbidities are case managed. This involves an ongoing problem solving approach aiming to anticipate, coordinate and join up health and social care.
- Provides expert support to GPs, practice nurses and other community staff on a consultancy basis.
- Education of staff – formal education programme or 1:1 in PCT clinic/GP setting or patient’s home.
- Ensure patients receive care as outlined in the CHD NSF/NICE guidelines in general practice.
- Provide a telephone cardiac advice service to patients, carers, and healthcare professionals during office hours.
- Advice on clinical strategic care for delivery or primary and secondary prevention strategies.
Criteria
Exclude:
- Patients under the age of 18.
Referral Process
Referrals need to contain patient demographic details, diagnosis and date of event, PMH and current medication. Diagnostic tests are also useful.
